Forms
Forms must be submitted at least four weeks prior to Prelims. All Forms must be given directly to Lindsay. Do not send any of these forms to the Graduate Division.
Requirement Completion Certificate:

Email Lindsay requesting a Requirement Completion Checklist. The process of reviewing your requirements can be started as soon as you complete your coursework. Complete the checklist and submit it to Lindsay. She will review the checklist and provide you with the Requirement Completion Certificate.
Enter all requested information. Enter “not applicable” in the section on language examinations. Then list your doctoral committee members alphabetically. If you have co-chairs, add “Co-” to the “chair” designation on the first committee member line. Then, next to “signature” on the next line, add the words “Co-Chair." Your committee chair/advisor does not sign this form. Please be sure to double check each committee member's academic rank and list the most up to date rank on the form.
Please print the SAMPLE announcement and use this as a guide. Be sure to accurately list your committee member's full title (i.e., If committee member is a Professor in Residence, this entire title must be listed on the announcement.) Completed announcements will be posted one week prior to your exam once all requirements have been completed and verified and your committee has been officially approved by the Graduate Division. You are responsible for turning in this form to Lindsay so that she can post it appropriately in the department.

Be sure to reserve Franz 3463 with Lindsay at least 4 weeks in advance of your prelim exam. If the room is already booked, she will work with you to find another room.
You are responsible for reserving a digital projector for your exam. You can make this reservation in the Psych Storeroom in A225. Remember to reserve it in advance.
Lindsay will prepare the Report on the Prelimary Oral Exam forms for your exam. Once you exam is completed, she will deliver them to the Graduate Division. The day that the form is submitted to the Graduate Division is the official day that you advance to candidacy.
Providing refreshments for your exam is not an obligation, but students may wish to supply some modest snacks and coffee is provided by the department. Consider using a buddy system, where a friend agrees to assist you by preparing snacks for your exam so you can concentrate on your presentation.
You will be charged a fee for advancing to candidacy. As of Fall quarter 2011, the total fee was $90. Be sure to watch you Bruin Bill account as this charge will confirm that your paperwork has gone through and been processed by the Graduate Division. If you do not see this charge promptly, please inquire with Lindsay.
